The structure and design of this course is to enable participants to understand their professional roles and responsibilities in the context of Person-Centred Care and Support Planning. This course will support participants to explore the assessment process intrinsic to support planning. In addition, participants will examine how a Support Plan document should in turn, reflect the holistic support or care needs assessed and those ordinarily involved in this process; with the person in receipt of support being central, from the outset.
Course Aims
- An introduction and overview to Person-Centred Care and Support Planning
- Roles and responsibilities of social care practitioners, in the support planning process
- The course aims to trigger thought, and enable participants to make formal correlations between assessing needs, assessing risks and support planning; including rights vs. risks and positive risk taking.
Course Objectives
By the end of the course participants will;
- Have a greater understanding of the importance, in assessment and support planning procedures, and in doing so will have an opportunity to explore key principles in reference to assessment and support planning.
- Acquire knowledge of others ordinarily involved in this process, outside of the organisation i.e. professionals/family members
- Participants will have an opportunity to examine the importance of continuity in the delivery of service provision; critical in meeting the needs of those in receipt of support/care
- Develop an appreciation for what a good Support Plan might look like; a centralised document, to reflect the support needs of the individual and how need should be met.
- This course will require participants to engage in a number of role play/group type activities; to support a more therapeutic and active learning approach.
